What is a
Community Health Worker?

The Department of Health Care Services (DHCS) added Community Health Worker (CHW) services as a Medi-Cal benefit starting July 1, 2022. There is no cost for Community Health Worker services.

 

A Community Health Worker is not a licensed medical professional and will not provide Diagnosis, Medical Treatment, or Prescribe Medication. ​A Community Health Worker is someone who helps people stay healthy and get the care they need. They often come from the same community as the people they help. 

 

Common Services a Community Health Worker Provides:

  • Helps people understand health information.

  • Explains medical instructions, paperwork, and health topics in simple words.

  • Connects people to services.

  • Helps find doctors, clinics, insurance, food programs, housing help, or transportation.

  • Supports doctor visits.

  • Helps schedule appointments, reminds people of visits, and may go with them for support.

  • Helps manage health conditions.

  • Supports people with diabetes, high blood pressure, asthma, pregnancy, and more.

  • Provides health education.

  • Helps with forms and applications.

  • Assists with insurance, benefits, and other important paperwork.

  • Offers emotional support.

  • Listens, encourages, and helps reduce stress by connecting people to help.

  • Advocates for the community.

  • Speaks up for community needs and helps people understand their rights.
